Benjamin Mako Hill 014b80ee72 use placeholder contact details in template defaults ...

Replace Mako's real phone number, email, and website with placeholder
values to match the rest of the fake "John Hancock" example content.
This commit is for the public master branch only and should not be
merged into branches customized with real contact details.
